Momentum 3 Capacity Building Fund

Who can apply: Must be a not-for-profit (501(c)(3) or 501(c)(4)) operating at the local level, providing HRSN services to Medicaid Members, and be contracted with Healthy Alliance. Per NYS guidelines, health systems, clinics, FQHCs, for-profit entities, and some other provider types are not eligible. Health Homes, Care Coordination, and Care Management Organizations may be eligible under certain criteria (contact Healthy Alliance).

Deadline: Rolling application period; requests accepted anytime through July 31, 2026.
