Who can apply: Providers must contract with a Social Care Network lead entity, provide screening and/or services consistent with the SCN program, complete required training, and use the SCN information technology platform. Eligible Medicaid populations include high utilizers, Health Home enrollees, individuals with substance use disorder, serious mental illness, intellectual or developmental disabilities, pregnant and postpartum people, certain individuals recently released from incarceration, and high-risk children under 18.
