Community Health Improvement Grants (CommonSpirit Health)
Who can apply: Applicant must be an IRS 501(c)(3) nonprofit or have a 501(c)(3) fiscal agent; applicants and partners must have missions not inconsistent with CommonSpirit Health; projects must address one or more significant needs in the hospital's most recent CHNA/implementation strategy; projects must serve people within the hospital's service area; projects must run 9–12 months; proposals requesting $20,000+ must include collaborating partner organizations; ineligible: unrestricted general operating support, event sponsorships, activities less than 9 months, capital projects, lobbying/political campaigns.
Deadline: Documented window: June 17 - August 16, 2024 (example cycle). Key dates in that cycle: notification ~Dec 18, 2024; project period Mar 1, 2025–Feb 28, 2026; mid-year & final report dates listed in program materials.
