Pillar 3 - Leadership Development and Capacity Building

Who can apply: Supports training and mentorship, organizational development, strategic planning, technology, data or infrastructure improvements, and network or coalition coordination. Organizations may receive only one individual capacity grant every three years. Multi-year awards may be considered for broad network initiatives.

Deadline: For May 2027 awards: letter of inquiry due November 16–December 15, 2026, at 5:00 p.m.; invited proposal due February 1, 2027, at 5:00 p.m. The board meets in May 2027. HFNLC conducts two grant cycles per year.

Pillar 1: General Program and Operating Grants

Who can apply: Organizations providing direct services and programs that address health and social determinants of health in northern Lake County. Smaller grants may be considered for grassroots or emerging organizations. Multi-year funding is generally reserved for long-standing grantee partners with strong performance history.
