Equitable Health Outcomes (formerly Health of the Urban Community)
Who can apply: Projects should focus on vulnerable populations, have the potential to reach large populations through existing community infrastructure, and may involve partnership with public and private funding sources. The Foundation favors proposals addressing prevention, detection, and early intervention. Organizations must be 501(c)(3) tax-exempt by the IRS.
Deadline: Quarterly deadlines; next proposal deadline listed as July 1, 2026. Other listed deadlines: January 5, 2026; April 6, 2026; October 1, 2026.
