Who can apply: Eligible organizations must reside in and serve communities within Arizona. Donations are limited to one request per organization per calendar year, and oral health instruction, screenings, or other dental health services must be provided with distribution.
Deadline: Requests must be submitted at least one month before supplies are needed; large requests require a minimum of six weeks for processing.
