¿Quién puede postularse? Must be a not-for-profit (501(c)(3) or 501(c)(4)) operating at the local level, providing HRSN services to Medicaid Members, and be contracted with Healthy Alliance. Per NYS guidelines, health systems, clinics, FQHCs, for-profit entities, and some other provider types are not eligible. Health Homes, Care Coordination, and Care Management Organizations may be eligible under certain criteria (contact Healthy Alliance).
