Small Grant Program — Community Health Spark Grant (up to $7,500)
¿Quién puede postularse? Applicants must be located in Grayson County, Kentucky, or demonstrate that funds will benefit Grayson County residents. Applicants generally must be tax exempt under IRC section 501(c)(3) or 170(c) and classified as a public charity under section 509, or provide a Board-identified unmet healthcare need. Applicants may generally request funding only once per calendar year; previously funded applicants must submit a final evaluation report. Restrictions apply to principally religious purposes, most pass-through grants, fundraising events, major medical research without qualifying collaboration, major arts organizations, political activities, international organizations, annual giving campaigns, and 509(a)(3) Type III supporting organizations.
Fecha límite: Applications are accepted during quarterly submission periods: January 1-March 31, April 1-June 30, July 1-September 30, and October 1-December 31. Award notifications are made by May 31, August 31, November 30, and February 28, respectively.
