Prospective Providers / Network Service Provider Contracting

Must be a nonprofit organization and must not be barred, suspended, prohibited from doing business with any government entity, under criminal investigation or indictment for conduct affecting ability to provide services or handle public funds, have had a contract terminated by the Department or CFBHN for failure to satisfactorily perform or for cause, have failed to implement an approved corrective action plan, or be ineligible for contracting under s. 215.473(2), F.S.
