LGBTQ+ health and care delivery don't share a funder profile very often. These two rolling programs sit at opposite ends of the scale, but both accept unsolicited proposals right now.

1. The Lazy Bear Fund, Lazy Bear Fund Grants

Up to $100,000 | United States and abroad | Rolling

lazybearfund.org

What we know: The Fund distributes proceeds from Lazy Bear Week to nonprofits serving LGBTQ+ communities across health, housing, youth services, arts, HIV/AIDS support, and mental health. Recent awards include $100,000 to Lyric SF and $50,000 to Positive Images of Sonoma County. The board reviews applications directly with no stated deadline.

Realistic fit: LGBTQ+ service organizations with concrete program work in health, housing, or youth, not advocacy-only shops.

2. The Commonwealth Fund, Improving Care Delivery

$37,040 to $453,583 | United States | Rolling LOI (grants above $200K go to board review in April, July, or November)

Funder profile: kindora.co/funders/131635260

What we know: The Commonwealth Fund ($923M in assets) funds research, pilots, and policy work to improve how care reaches patients. Foundation-wide, its top recipients are research-affiliated: Harvard, Urban Institute, and Georgetown together account for over $11M across 83 grants. About 48% of all grants go to organizations in New York and Washington, D.C.

Realistic fit: policy shops, research universities, or health systems with a named PI and a care-delivery reform angle.

The Commonwealth Fund's LOI process is structured: summarize the problem, design, cost, and outcomes before you're invited to submit a full proposal. Worth reviewing their 2026 priorities page before drafting. If you work with an LGBTQ+ health nonprofit or a care-delivery research team, send this their way.
