Who can apply: Applicants must be a 501(c)(3) nonprofit or government organization serving at least one of the 12 counties of focus. Organizations without 501(c)(3) status may apply through a fiscal sponsor, subject to case-by-case review and prior contact with PHFO. Organizations with an active HealthPath Foundation of Ohio or Public Health Fund of Ohio grant are ineligible until the current grant period concludes. Applicants may submit one application per funding opportunity. Funding excludes capital campaigns, healthcare levies, lobbying or political campaigns, emergency needs, scholarships, endowments, and grants to individuals.

Deadline: Applications close October 6, 2026, at 11:59 p.m. EST. Decisions are expected by November 20, 2026, and grant payments are expected by the end of January 2027.

Who can apply: Applicants must meet the eligibility requirements in the RFP and generally be organizations based in and serving HealthPath's 36-county service area. HealthPath generally funds 501(c)(3) nonprofit organizations and governmental agencies; eligible community-based organizations may apply through fiscal sponsorship.
