Rehab Funding Program

Who can apply: For people currently in rehab who are about to reach their therapy cap and need continued rehabilitation. The process begins when a doctor or current therapist contacts a partner rehab location for referral; the applicant then completes an application, including disclosure of household income. The foundation only partners with nonprofit rehabilitation centers.
