    Who can apply: Applicants must be IRS 501(c)(3) nonprofit organizations or work through a capable 501(c)(3) fiscal agent, provide an IRS determination letter and be in good standing. Applicants and partners must have philosophies and services consistent with CommonSpirit Health's mission, vision and values. Projects must address significant health needs identified in the relevant hospital's most recent CHNA or Community Health Implementation Strategy, serve people within the hospital's service area, and include underserved or vulnerable populations. Projects must run for 9 to 12 months and are limited to one-year periods. Projects requesting $20,000 or more must include collaborating partner organizations with distinct, complementary and substantive roles. Unrestricted general operating support, event sponsorships, projects shorter than nine months, capital projects, lobbying, political advocacy and political campaigns are ineligible. Projects may not be funded for more than four consecutive years.

    Deadline: 2027 grant cycle applications accepted July 20 through September 18, 2026. Applicants are expected to be notified on or about January 20, 2027. Grant agreements and payments occur February-March 2027; projects run April 1, 2027 through March 31, 2028.
