About this program
Mouthguard donation program providing free, high-quality mouthguards to middle and high school athletic programs throughout Illinois. Schools request mouthguards via an online form; distribution is free while supplies last.
Who can apply
Open to middle and high school sports teams in Illinois. Only official school representatives (coaches, athletic directors, administrators) can submit requests. Each school can enter once per sport.
